Job Description Job Description Job Summary: We are looking for a dedicated and compassionate Physical Therapist Assistant (PTA) to join our Home Health team. In this role, you will work under the supervision of a licensed Physical Therapist to provide rehabilitative services to patients in their homes. You will help patients improve mobility, manage pain, and regain independence in the comfort of their own environment. Key Responsibilities: Implement physical therapy treatment plans developed by the supervising Physical Therapist. Visit patients in their homes to provide therapeutic exercises, gait training, and functional mobility interventions. Monitor and document patient progress, reporting changes to the supervising therapist. Educate patients and caregivers on home exercise programs and safety techniques. Ensure compliance with all regulatory and documentation requirements. Maintain accurate and timely clinical records using electronic health record (EHR) systems. Communicate effectively with patients, families, and interdisciplinary team members. Qualifications: Associate degree from an accredited Physical Therapist Assistant program. Current state licensure or certification as a Physical Therapist Assistant. Valid driver’s license and reliable transportation. CPR certification. Previous experience in home health or geriatric care preferred. Skills and Competencies: Strong interpersonal and communication skills. Ability to work independently and manage a flexible schedule. Knowledge of safety and infection control practices in home settings. Work Environment: Travel to patient homes within a designated geographic area. Exposure to various home environments and conditions. May require lifting, transferring, and assisting patients with mobility.
